WHAT YOU WILL DO Make initial nursing evaluation visit to
communicate with physician and determine a plan of care.
Practice nursing according to Franciscan Alliance and nursing
division's philosophies and standards of care set by state board of
nursing, state nurse practice act, and other governing agency
regulations.
Establish a compassionate environment by providing emotional,
psychological, and spiritual support to patients, friends, and
families.
Collaborate with patients and their families, as well as members of
a multidisciplinary team, to ensure optimal patient outcomes and
enhancement of the patient experience.
Assess, collect, and analyze comprehensive objective, subjective,
psychosocial, and cultural data pertinent to patient condition to
determine the nursing diagnosis, patient needs and plan of
care.
Develop an appropriate plan of care based on objective and
subjective data, observations, and communications from patient and
family and continuously review and revise the plan of care as
patient condition warrants.
Communicate timely changes in patient's assessment/ongoing
reassessment to providers that warrant consideration and/or a
change in the plan of care and maintain and coordinate
communication with all members of the multidisciplinary team to
facilitate safe transitions and continuity in care delivery.
Supervise clinical care provided by LPNs and Home Health Aides.
Serve in an on-call rotation and provides patient care outside of
regular business hours.
WHAT IT TAKES TO SUCCEED Minimum one year of prior Acute Care, Home
Health and/or Long Term Care experience.
Develop own ways of working effectively and efficiently.
Handle multiple responsibilities simultaneously.
Accept personal accountability for deliverables and actions.
Develop effective relationships with patients, hospital
departments, and other external parties.
Interpret test results and formulate reports.
Educate patients and/or families as to the nature of disease and to
provide instruction on proper care and treatment.

Prioritize and delegate tasks.
Show sincere interest in others and their concerns.
