Job Description:
The Director of Case Management is responsible for the overall
facilitation of hospitalization from admission through discharge.
The Director will coordinate with Physicians, Nurses, Social
Workers and other health team members to make sure processes are in
place to expedite medically appropriate, cost-effective care. The
Director develops guidelines for the Case Managers to apply their
clinical expertise and medical appropriateness criteria to resource
utilization and discharge planning. The Director will mentor the
Case Management team and provide leadership as
needed.Education:Associate or Bachelor's degree in Nursing from an
accredited college or university. Diploma Required Current Indiana
Registered Nurse licensureBachelor's or Master's degree in related
field preferredCCM certification
preferredCertifications/Experience:Preferred three to five years in
acute clinical nursing and five years in a leadership position in a
healthcare setting.Preferred three years acute care case management
experience preferred General ResponsibilitiesEffectively plans and
budgets activities of the departments managed.Works closely with
Medical Staff Office and Performance Measurement and Enhancement
Department to assist in the development of clinical
pathways.Identifies, reports and provides recommendations for
facility quality improvement and revenue enhancement opportunities
directly related to patient case management and identified
operational processes.Maintains effective relationships with
medical staff and physician advisors.Appearance - Takes pride in
