Registered Nurse (RN) | Home Health Location: New Lenox, IL Agency: Cynet Health Pay: $2,216 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/16/2026 About the Position TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in New Lenox, Illinois, 60451! Job Title: Registered Nurse (RN) Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8:00 AM - 5:30 PM Experience: Minimum years of experience required License: Valid nursing license required Certifications: BLS required Must-Have: Home Health experience required Valid driver's license and car insurance required OASIS and start of care experience strongly preferred COVID and flu vaccination required Description: Prescribes, coordinates, and delivers care through the nursing process. Maintains the standards of professional nursing. Supervises CNA and coordinates care with other healthcare workers as assigned. Provides comprehensive initial assessments to all clients and, on subsequent visits, continually assesses physical, psychosocial, environmental, and community resources available. Utilizes the principles of growth and development over the lifespan. Initiates Plan of Care, re-evaluates the need for care, and modifies Plan of Care as ordered or needed. Establishes discharge plans with the patient, physician, other disciplines, and caregivers. Completes OASIS data. Provides patient care using current federal, state, and agency regulations. Assesses educational needs of patients, families, and caregivers, and implements appropriate teaching strategies. Completes documentation according to regulatory guidelines. Promotes a clean and safe environment of care, utilizing error prevention habits.
