Shift: 8:30AM-5:00PM CST. Location : Shelters City Wide | Health
Centers EW and Broadway Heartland Alliance Health’s (HAH) mission
is to transform healthcare for the most vulnerable – particularly
people experiencing homelessness, mental illness or addictions, or
struggling with multiple chronic illnesses – improving health for
all and the well-being of our community. The Community Dietitian
provides one on one counseling to participants of the health and
grocery centers. The Dietitian will provide medical nutrition
therapy to patients living with an array medical conditions. The
population includes individuals who may experience homelessness,
living with HIV/AIDS, refugees, living with mental health
conditions or engage in substance use. Essential Functions: Duties
& Responsibilities Conduct comprehensive nutritional assessments
for participants at health and grocery centers. Develop, implement,
and monitor individualized, evidence-based nutrition care plans.
Document the nutrition care process accurately in electronic
medical records. Deliver group nutrition education sessions at
health centers, grocery centers, and community events. Create and
distribute educational materials across multiple platforms to
engage participants. Collaborate with community partners to support
and implement public health initiatives. Utilize agency-approved
tracking systems to record participant encounters and process
service invoices. Provide operational support to grocery centers as
assigned. Qualifications: To perform this job successfully, an

Bachelors Degree in Nutrition and Dietetics or related discipline
required Certifications/Licenses: Registered Dietitian required
Licensed Dietitian Nutritionist in Illinois required Food
protection management preferred Knowledge/Skills/Abilities (K/S/A):
Demonstrated proficiency in staying current with evidence-based
research in nutrition and disease management Access to regular,
dependable and timely transportation Ability and comfortability
working with people of varying socioeconomic backgrounds. Excellent
communication skills. Strong networker, convener, relationship
builder and communicator with experience collaborating with diverse
work teams and stakeholders. Demonstrated ability to handle
confidential and sensitive information possessing skills to handle
situations in a professional and diplomatic manner. A high degree
of collaboration and emotional intelligence, self-motivation, and
analytical ability. Proficiency in Microsoft Office, various
internet browsers, Outlook/Exchange; Windows operating systems; and
other software routinely used by Heartland Alliance Health Ability
to solve practical problems and deal with a variety of concrete
variables in situations where only limited standardization exists.
Ability to interpret a variety of instructions furnished in
written, oral, diagram, or schedule form. Ability to work
independently and remain self-motivated with minimal direct
supervision. Strategic thought partnership and consultancy skills.
