Bachelor's, Master's or Doctoral Degree in Physical Therapy from an
accredited program approved by the APTA. Active Illinois PT license
required to start the assignment. BLS certification required from
the AHA or ARC. Minimum 1 year of recent clinical experience in
outpatient, home health, or skilled nursing settings preferred;
client may require home health experience. Experience with
documentation in electronic medical record systems preferred. Valid
driver’s license and reliable transportation required for home
health travel between patient homes. Assignment Details Facility
Type: Home Health. Shift Name: Days. Candidates must have reliable
transportation for travel to patient homes. May require floating to
