Job Description:
A company is looking for a Supervisor, Care Coordination.
Key Responsibilities:
Supervises care coordination activities to ensure appropriate
services are delivered by the team and providers
Monitors and reviews care coordination documentation for compliance
with contractual requirements and standards
Educates and provides guidance to the care coordination team to
address member and provider issues
Required Qualifications:
Bachelor's degree and 4+ years of related experience
Experience in evaluating team performance and providing
feedback
Ability to oversee telephonic, digital, and other outreach to
assess member needs
Experience in onboarding, hiring, and training team members
Knowledge of ensuring adherence to service level, quality, and KPI
expectations
