Job Description:
A company is looking for a Remote Bilingual LPN to support
patients in chronic care management and remote monitoring
programs.
Key Responsibilities
Conduct virtual consultations with patients focusing on education
related to chronic diseases
Review and respond to remotely transmitted patient data such as
blood pressure and glucose levels
Document patient communication and assist patients in setting up
their monitoring devices
Required Qualifications
Fluent in English and Spanish, including reading and writing
A current, valid, and in good standing Multistate/Compact Nursing
License (LPN/LVN/RN)
3+ years of experience in cardiology, primary care, home care, or
chronic care management
Experience in remote monitoring and/or virtual care management
programs (preferred)
Must have a HIPAA-compliant and distraction-free remote work
environment
